Has your company recently changed name or under new ownership? Please complete this form and return it to us along with your change of name certificate so we can update our records.

 

Would you like to change the account from where payments are made from or Direct Debits are collected in to? Please complete and return this form to us and we will update our records. Would you like to make a change to where we send the money collected from your customers? Please complete and return this form to us.
